About Us

Hospice is a concept of care derived from medieval times, symbolizing a place where travelers, pilgrims and the sick, wounded and dying could find rest and peace. Today, Hospice Care offers a program of care for patients facing terminal illness. The focus of care is quality of life and comfort care delivered compassionately by professionals who specialize in end-of-life challenges.

When it’s Time
Anyone can refer a patient for hospice care. If the referral is made by a family member or friend, the CCH Admission Team will contact the patient’s physician to ensure that the patient qualifies for Hospice Care. To qualify, the patient’s physician and the hospice medical director must certify that the patient has a terminal condition with a life expectancy of six-months or less, if the disease runs its normal course.
Crater Community Hospice provides care where the patient lives (home or nursing facility) or in a hospital setting. The care is provided by a team of hospice professionals and volunteers who provide physical, emotional, psychosocial, and spiritual support to the patient and family. The CCH Care Team includes a physician, a nurse, a hospice aide, a social worker, chaplain, a bereavement counselor and trained volunteers.
CCH accepts most insurance plans including Medicare and Medicaid. The hospice benefit provides all medically necessary equipment and appropriate medications for the patient’s specific disease process and accompanying symptoms to maximize their comfort
